The PAY Case Manager supports participants aged 12-21 in completing community service hours and achieving personal goals. They coordinate youth program activities and build partnerships with community organizations.
PRIMARY DUTIES AND RESPONSIBILITIESDuties and responsibilities of the Pay Case Manager include but are not limited to:
Administrative and Program related- Coordinates, communicates and monitors participants’ case plan to encourage successful completion of program.
- Oversees scheduling of youth in the program.
- Documents all contacts and information as per guidelines.
- Tracks youth’s attendance.
- Completes monthly reports.
- Liaises with community organizations to develop partnerships based on program and participant needs.
- Implements and adheres to all agency and program policies and procedures.
- Identifies and communicates concerns and updates regarding community programming.
- Attends and participates in program meetings.
- Addresses work centre concerns with appropriate personnel when necessary and in a timely manner.
- Ensures all adult volunteer mentors, students and new staff receive support, training and direction.
- Supervises mentors while working in the community.
- Provides feedback and evaluation to mentors, and students.
- Builds positive relationships and rapport with participants, helping to identify personal strengths to promote a sense of belonging, independence, generosity and mastery.
- Acts as an advocate on behalf of the best interest of youth, ensuring their rights are respected.
- Makes links and connections with participants in the community to support their skill development utilizing volunteer work and participant goals.
- Acts as a positive role model for youth.
- Ensures all participants receive support, encouragement and follow-up.
- Manages case load, including intake, scheduling and follow up.
The successful candidate should demonstrate the following competencies, which incorporate the Seven Sacred Teachings;
- Post‑secondary education in relevant social services or a combination of work and/or life experience.
- Demonstrated knowledge of cultural awareness, anti‑oppressive and trauma informed best practices.
- Knowledge and experience with diverse cultures and socio‑economic backgrounds.
- Knowledge of Strength‑based philosophy and solution‑oriented approach. Understanding of crisis intervention, harm reduction framework and conflict resolution skills.
- Ability to work independently as well as within a team.
- Demonstrated experience in group facilitation.
- Strong interpersonal skills.
- Ability to be flexible with schedule based on program and participant needs (days, evenings and weekends).
- Willingness to obtain necessary training as per the organization’s training policy including Crisis De‑escalation, Applied Suicide Intervention, Colonization Awareness, and Child Development.
- Emergency First Aid and CPR Level C, or willingness to obtain.
Satisfactory Criminal Record Check with Vulnerable Sector Search, a clear Child Abuse Registry check with acceptable Prior Contact checks, and a clear Adult Abuse Registry check, all done within the last 3 months. - Valid driver’s license and acceptable driver’s abstract (subject to approval).
- Access to a personal vehicle for work is required.
$20.05 - $23.86 per hour. Depending on the qualification.
Hours of Work37.5 hours per week – combination of days, evenings, and weekends. 12‑month term.
