• Assist clients in navigating healthcare and social services

• Collaborate during case conferences with interdisciplinary teams

• Keep accurate documentation and manage community referrals

Requirements:

• Degree in Social Work and full registration in BC

• Two years' of experience working with youth populations

• Knowledge of trauma-informed care practices is essential

• Proficient in group facilitation and clear communication

• BC Driver's License required for local traveling
