Position: Outreach Worker for HIV Support
Join Nine Circles Community Health Centre as an Outreach Worker, focusing on support for clients living with HIV in Winnipeg. This full-time role offers the chance to make a real impact in community health.

As part of the Outreach Program, you will address complex health needs with clients and engage collaboratively with the Social Work Team. The role incorporates various work settings, emphasizing health service utilization and client empowerment, with potential evening and weekend hours required.

Key Responsibilities:

• Deliver outreach services in clinics and community settings

• Support clients in navigating health systems

• Work within multidisciplinary teams for client care

• Prepare client documentation accurately

• Provide services in client homes and shelters

Requirements:

• Certificate in Social Services or related field

• At least three years of experience in health-related roles

• Clean driver's abstract and valid Manitoba driver's license

• Strong understanding of social determinants of health

• Exceptional communication and interpersonal skills
