As a Peer Outreach Worker, you will use your background to connect with clients, assess their needs, and provide critical support services. This role operates within the Van Outreach Services, ensuring clients receive resources and care. You'll work collaboratively to implement effective care plans and promote a supportive environment.
Key Responsibilities:
• Assess and fulfill the immediate needs of clients
• Conduct community outreach and support teams
• Gather data for client assessments and care planning
• Operate mobile outreach services effectively
• Maintain accurate and confidential records
Requirements:
• First-hand understanding of mental health challenges
• Valid Class 5 Driver's License
• Ability to interact with diverse populations
• Experience with statistical reporting
• Clear Criminal and Adult Abuse Registry Check
