Provide essential social work services at Southern Health–Santé Sud as a Personal Care Home Social Worker. Engage with residents and families while advocating for their needs in a team‑oriented workplace.

The Person al Care Home Social Worker will be part of a multi‑disciplinary team, responsible for counseling, consultation, and education. In this role, you will identify concerns within the facility and cooperate with staff to implement interventions for residents’ psycho‑social needs. You should demonstrate independent judgment and initiative while prioritizing work tasks effectively.

• Deliver social work services to residents and families
• Provide counseling and advocacy for individuals
• Collaborate with multi‑disciplinary team members
• Identify psycho‑social concerns and implement interventions
• Maintain confidentiality in all forms of documentation

• One year of experience as a Social Worker in long‑term care
• Graduate of an accredited Faculty of Social Work
• Current registration with Manitoba College of Social Workers
• Bilingual proficiency in English and French
• Valid Class 5 driver’s license and insured vehicle
