Sacred Care Case Manager

In this full-time, permanent role, you will engage with families and collaborate with a dedicated support team. Your responsibilities include conducting visits, facilitating meetings, and ensuring compliance with licensing documentation. Emphasizing First Nations philosophies, you will play a critical role in delivering holistic support.
Key Responsibilities:
• Conduct regular home visits for relationship building
• Host monthly case planning meetings in foster homes
• Complete essential licensing and documentation tasks
• Assess and provide updates on household services
• Collaborate on training needs for caregivers
Requirements:
• BSW degree or equivalent experience preferred
• Knowledge of foster care standards and legislation
• Proficient in Microsoft Office for documentation
• Strong communication and organizational skills
• Valid driver's license and reliable vehicle required
