Care Planner

Background

Ma Mawi Wi Chi Itata Centre is seeking an individual for the position of Care Planner. The candidate will facilitate and support, providing case management for at-risk females and transgender youth aged 13-17 years of age. The site is a six-bed facility operating 24/7 with a range of assessment and support services. All six beds are anticipated to run at high occupancy in short-term and long-term residency.

The facility provides a safe, nurturing environment where young women can experience care, support, encouragement, and build capacity to achieve their dreams.

• Child & Youth Care Diploma or acceptable equivalent and related experience;
• Residential Care experience would be considered an asset;
• Caseload and healing plan development and implementation with young females;
• Provides support/supervision to residents;
• Program development and facilitation skills;
• Liaises, supervises and consults with collateral agencies about specific service recipients or service concerns;
• Ability to work cooperatively with others and contribute as a team member;
• Ability to work under stressful situations;
• Knowledge of Child Welfare Legislation and Standards;
• Strong verbal and written communication skills;
• Understanding of Indigenous values and community issues;
• Current First Aid/CPR certification;
• Valid Manitoba Class 5 Driver’s License required;
• Current (within the last three months) Criminal Record Check with Vulnerable Sector Query, Child Abuse Registry Check and prior contact check.
