The Community Housing Transition Worker, under the Lead Service Coordinator's guidance, plays a crucial role in helping clients navigate their paths to permanent housing. You will conduct interviews, assess skills, and assist clients in developing action plans. This role involves organizing social development workshops and collaborating with various community resources to provide comprehensive support.

Key Responsibilities:

• Interview clients for case history and information

• Assess client skills and strengths for housing options

• Assist clients in locating essential community resources

• Conduct workshops on life skills and personal development

• Track client progress and evaluate treatment program effectiveness

Requirements:

• Diploma in social work, counseling, or related field

• Experience in a social service environment

• Knowledge of trauma-informed care and harm reduction

• Excellent communication and interpersonal skills

• Valid Class 5 Driver’s License and reliable vehicle
