About The Company
Main Street Project (MSP) is a community health Centre with a mandate to address the social determinants of health, including addiction, physical and mental health issues, and homelessness. MSP uses housing‑first and harm‑reduction principles to provide a safe, respectful and accessible place for individuals experiencing or at risk of homelessness.
Since 1972, MSP has offered emergency shelter, nutrition services, drug and alcohol withdrawal management, health advocacy, case management, housing support, food bank services, and a mobile community outreach program.
Job SummaryThe case management team is a key connection to outside organizations and participants seeking services. The Housing Case Manager works from a housing‑first, harm‑reduction, and trauma‑informed lens to support people in finding and maintaining housing, connecting to primary care, income, addiction, and mental health supports. Relationship building is the core function of this team.
Key Responsibilities- Provide ongoing case management support, assess progress, and ensure treatment plans are followed.
- Develop and coordinate individualized care plans addressing short‑term and long‑term goals collaboratively with participants, interdisciplinary team members, and external partners.
- Assess an unsheltered person’s needs and proactively identify barriers that restrict participants’ ability to meet identified goals.
- Create and maintain a professional relationship with community members, including boundary setting.
- Keep accurate, continuous, legible, and signed records, including progress notes, checklists, correspondence, and journals in accordance with established protocols.
- Work with involuntary participants and participants not capable of consent.
- Apply ethics of case management, including confidentiality, self‑determination, informed consent, and addressing power differentials.
- Plan processes and interventions with participants.
- Identify and provide information and resources to participants, including:
- Community resources
- Rent and income assistance
- Mental health resources
- Indigenous organizations
- Establish partnerships with landlords and service agencies, advocating on behalf of participants.
- Identify symptoms of suicide ideation and offer resources and support.
- Create environments of cultural safety for participants.
- Meet with participants both onsite and in the community.
- Monitor, evaluate, and communicate progress of participants with participants, team members, and managers on a regular basis.
- Maintain accurate and confidential electronic and file records, including statistical data.
- Conduct crisis intervention as needed.
- Attend program‑related community, coalition, and committee meetings.
- Build effective relationships with encampment residents.
- Coordinate with local agencies to provide services that meet the needs of residents.
- Contribute to the development of a community that values cultural diversity.
- Preferred experience with homeless outreach working with homeless individuals/families, proven ability to access a wide range of homeless services, and familiarity with overcoming the barriers homeless people encounter.
- Provide a supportive and proactive approach to eviction prevention.
- Provide an open and welcoming environment for tenants to share their housing concerns and address tenants housing needs.
- Review client risks or issues and utilize a team approach to identify action plans.
- Experience with palliative and after‑death care.
- Experience with harm‑reduction in a supportive living environment.
- Provide comprehensive case management services to families affected by MMIWG, demonstrating sensitivity to cultural nuances and the diverse needs of Two‑Spirit individuals.
- Establish and maintain supportive relationships with families, acting as a liaison between them and relevant law enforcement agencies, social services, and other community resources.
- Facilitate cultural ceremonies, healing circles, or support groups to address the emotional and spiritual needs of affected families and individuals.
