As a key member of the Van Outreach team, you will build trust with individuals facing homelessness and mental health challenges.
Your role involves comprehensive assessments, advocacy, and the delivery of essential supplies, supporting clients' safe transitions toward recovery.
Key Responsibilities:
• Meet clients’ basic needs with necessary supplies
• Assess behavior and provide safety monitoring
• Conduct assessments and engage in case management
• Collaborate with community partners for support
• Follow up with new encampments and community requests
Requirements:
• Relevant personal experience with homelessness or mental health
• Clean Class 5 Driver's License
• Experience in staff training and statistics preferred
• Proficiency in electronic record-keeping methods
• Must pass background checks for vulnerable sectors
