Position Summary
The Independent Living Transition Facilitator (ILTF) supports youth in developing the practical life skills, confidence, and community connections necessary for a successful transition to independent adulthood. Through mentorship, skill development, advocacy, and individualized support, the ILTF empowers youth to achieve greater self‑sufficiency, stability, and personal growth while fostering resilience and positive outcomes.
This position works primarily with youth who may have experienced trauma, involvement with child and family services, housing instability, mental health challenges, or other barriers to independence. The ILTF utilizes a strength‑based, trauma‑informed, and culturally responsive approach.
Duties and Responsibilities
Provide coaching and mentorship in budgeting, financial literacy, household management, and tenancy responsibilities
Support youth in meal planning, grocery budgeting, nutrition education, and food preparation
Assist youth in accessing education, employment, housing, healthcare, and community resources
Develop and monitor individualized goals and transition plans
Build trusting relationships that foster resilience, confidence, and independence
Maintain accurate electronic documentation and reports
Qualifications
Prefer a minimum of two years of experience working with at‑risk youth or vulnerable populations
Certificate/diploma in Child & Youth Care, Social Work, Psychology, Counseling, Human Services, or related field preferred
Valid First Aid and CPR certification
Knowledge of trauma‑informed, strengths‑based, and culturally safe approaches
Valid Class 5F Driver’s License and reliable vehicle
Knowledge of community resources and support for youth transitioning to Independent Living
