Position: Youth Transition Facilitator for Independence
Empower vulnerable youth at their journey to independence with an Independent Living Transition Facilitator role. Support their growth through mentorship, advocacy, and community connections in a culturally responsive manner.

As an Independent Living Transition Facilitator, you will work with youth who face various challenges, helping them to develop essential life skills.

Your role focuses on coaching in financial literacy, household management, and resource access, with an emphasis on resilience and personal growth. Building trusting relationships and developing individual transition plans are central to your impact.

Key Responsibilities:

• Provide mentorship in budgeting and financial literacy

• Support meal planning and nutrition education

• Assist in accessing education and community resources

• Develop and monitor individualized transition goals

• Maintain accurate electronic documentation and reports

Requirements:

• Minimum of two years’ experience with at-risk youth

• Preferred degree in Child & Youth Care or related field

• Valid First Aid and CPR certification

• Knowledge of trauma-informed practices

• Class 5F Driver’s License and reliable vehicle

Contribute to the self-sufficiency and stability of youth through dedicated support and skill development.
