Empower youth to build life skills as an Independent Living Transition Facilitator. Support their journey to self-sufficiency through mentorship, advocacy, and community connection.
In this rewarding role, the Independent Living Transition Facilitator focuses on nurturing youth facing barriers to independence. You will provide guidance in budgeting, meal planning, and accessing essential resources, helping them to develop confidence and resilience. The position requires crafting individualized goals and fostering trusting relationships for positive outcomes.
Key Responsibilities:
• Coach youth in budgeting and household management
• Support meal planning and nutrition education
• Assist with accessing education and community resources
• Develop and monitor personalized transition plans
• Maintain accurate electronic documentation and reports
Requirements:
• Minimum two years working with at-risk youth
• Certificate in Child & Youth Care or related field
• Valid First Aid and CPR certification
• Knowledge of trauma-informed practices
• Valid Class 5F Driver’s License and reliable vehicle
Foster growth, stability, and independence for youth through your dedicated support as an ILTF.
