Under the direction of the Eagle Urban Transition Centre’s Director of Service Delivery, the
Jordan’s Principle Advocate/Support Worker must have wide range of knowledge and
experience working in, and with, the First Nations children, youth and families. The Jordan’s
Principle Advocate/Support Worker must be able to work both independently and within a
team environment to support in providing resources and referrals on behalf of First Nations.

• Support and assist clients and families in establishing a process for resolution of concerns
  and referral to key contacts (federal, provincial) or external as appropriate.
• Communicates to the clients/families the rationale for health care specific interventions,
  plans and learning needs and how they will be met.
• Involves the clients/families in developing the care and support plan.
• Communicates to the clients/families, as necessary, referral information that will be given
  to appropriate community organizations.
• Collaborates with and make referrals to appropriate service providers and or programs for
  resolution of issues of concern.
• Maintain confidentiality according to PHIA standards.
• Maintain confidentiality in preparing, opening and closing, a case file for clients.
• Consult, collaborate, and provide reports and data to Eagle Urban Transition Centre’s
  Jordan’s Principle Project Coordinator.
• Other duties as assigned.

• Post-Secondary degree in Social Work or equivalent training/certification.
• Equivalent combination of education and experience working in the child welfare system,
  working with First Nation individuals with complex medical conditions, disabilities and may
  be considered.
• Should be knowledgeable in the Personal Health Information Act (PHIA) structure and
  adhere to the policies.
• Excellent verbal, listening, written and inter-personal communication skills.
• Ability to work closely with health care executives and professionals, senior levels of
  government and community members.
• Experience working effectively with First Nation individuals, communities, and traditional
  knowledge systems as well as understanding First Nation health issues.
• Knowledge and familiarity of the programs and resources available to First Nations
  citizens and communities, on and off Reserve, including special needs and disability service&
• Knowledge and respect of the culture, traditions, attitudes, and conditions of the First
  Nation families, children and the communities.
• Experience in using Client Centered Case Management as a service delivery tool.
• Ability to establish and maintain positive, compassionate and effective work relationships
  with co-workers, clients, and external agencies.
• Demonstrates problem solving and organizational skills.
• Ability to use Microsoft Office software, Word, Excel, and Outlook.
• Excellent time management skills.
• Ability to work independently and with minimum supervision.
• Knowledge of First Nations culture and fluent in a First Nations language would be an
  asset.
• Ability to function effectively as a team member.
• Adhere to AMC policies, and procedures with specific attention to attendance, and
  professional ethics.
• Must have a valid Class 5 driver’s license and a reliable vehicle.
• Must have a current satisfactory Criminal Record Check, Clear Child Abuse Checks.

Preference will be given to First Nations people. If successful, applicants will be asked to verify citizenship documentation issued by a First Nation or government‑issued documentation or affirmation from the Inuit, Métis, or non‑status Indigenous community with which they identify.
