In this full-time Clinical Case Manager role, you will serve a diverse caseload, working directly with treatment foster care providers. Your responsibilities will include conducting clinical assessments, developing care strategies, and collaborating with various stakeholders to improve outcomes for children and youth. The position emphasizes a trauma-informed approach to support those at risk effectively.

Key Responsibilities:

• Monitor and supervise treatment foster homes through regular visits

• Educate and coach foster care providers in therapeutic practices

• Conduct clinical assessments and evaluations of care plans

• Develop and implement program operations ensuring compliance

• Recruit foster placements and perform intensive home studies

Requirements:

• Post-secondary education in Social Work or equivalent experience

• Minimum 4 years of relevant clinical work experience

• Strong understanding of mental health and trauma in vulnerable populations

• Valid First Aid/CPR certification required

• Experience in crisis de-escalation and behaviour management
