This permanent position involves collaboration within the Community Services - ACT department. As an Outreach Worker, you will engage proactively with clients, offering a variety of services aligned with individual needs.
Your role emphasizes relationship building and utilizes a client-centered approach, addressing social determinants of health and promoting health equity.
Key Responsibilities:
• Deliver support and recovery services based on individualized service plans
• Engage with new and existing clients to facilitate skill-building
• Advocate for clients using a non-judgmental and culturally appropriate manner
• Collaborate within an interdisciplinary team to achieve service goals
• Apply harm reduction and trauma-informed practices for client care
Requirements:
• Valid Driver’s License and access to reliable transportation
• Minimum 2 years’ experience with community programs
• Proven skills in crisis management and problem-solving
• Understanding of Indigenous cultures and their impacts
• Strong communication and computer proficiency
