Working within an interdisciplinary team, you will implement the Assertive Community Treatment Model and the Housing First Principles. This position involves helping clients achieve their goals through proactive outreach while respecting their unique backgrounds. You will provide culturally safe services, emphasizing the values of equity and social justice.
Key Responsibilities:
• Work with clients to establish and follow service delivery plans
• Offer support aimed at skill development and goal achievement
• Advocate for improved health outcomes in a respectful environment
• Participate actively in team collaborations for services
• Integrate trauma-informed care principles in outreach practices
Requirements:
• Driver’s License and reliable vehicle access required
• 2+ years in community service roles
• Demonstrated crisis management abilities
• Knowledge of Indigenous health issues
• Excellent written and verbal communication skills
