We are currently looking to fill the position of Community Health Representative with in our community. This position is combined with Community Oral Health Initiative, MST clerk, and Maternal Child Health

The desired applicant will possess the following knowledge, skills, and abilities:

Strong documenting, filing, and organizational skills

Attention to detail

Ability to prepare and maintain schedules and budgets

Able to stay calm under pressure

Ability to provide safe care for newborns and new parents (empathy and understanding, a calm demeanor)

Remain professional at all times and create a welcoming environment

Open to pursuing and completing relevant education and training requirements

Provide a recent criminal record check from Canadian Police Information Centre (CPIC).

Responsibilities

Maintain Confidentiality

Support and Advocate for healthy homes for newborns to reside in

Support community clinics and workshops

Arrange medical transportation for members

Prepare required reviews for members

Provide information and advice on health matters to individuals and families;

Make referrals to the Community Health Nurse when necessary;

Act as the liaison between community members and the Nurses in Health Department and Childrens Oral Health Initiative;

Provide Prenatal and Baby Clinics;

Support the Nurse in completing home visits for New Baby and Mother health; and

Promote and engage members participation in healthy activities.

Prepare budgets and submit to Health Director for approval

Education and Experience

Minimum High School Diploma, (4-5 years experience)

Preferred Post-Secondary Degree or diploma in Indigenous Peoples Health and Palliative Care

Working Conditions

Indoor work in an office and home setting

Travel is required to conduct home visits in partnership with the Nurse

Occasional heavy lifting
