Key Responsibilities:

• Maintain up-to-date participant files and records

• Document daily activities and system interactions

• Write incident reports with Clinical Case Management

• Support participants in goal-setting and planning

• Engage in other assigned duties as needed

Requirements:

• Minimum two years of direct support experience

• Valid Standard First Aid/CPR certification

• Clear criminal background checks

• Post-secondary education in Human Services preferred

• Excellent communication and crisis management skills
