Position: Part-Time Social Worker at Extendicare

Extendicare is seeking a dedicated part-time Social Worker to enhance client and family functioning within rehabilitation contexts. Key responsibilities include conducting pre-admission procedures, coordinating admissions, and leading social work programs tailored to residents' needs. Collaborate with a multi-disciplinary team to truly make an impact on the lives of vulnerable adults.

Key Responsibilities:

• Respond to inquiries regarding placement and lifestyle

• Conduct tours for prospective residents and advocates

• Coordinate new admissions with families and advocates

• Organize social work programs for resident psychological needs

• Participate in Resident Care and Family Conferences

Requirements:

• Registered Social Worker under the Health Professions Act

• Bachelor of Social Work from a recognized institution

• Experience working with Geriatrics

• Ability to work effectively in a multi-disciplinary team

• Must obtain a Vulnerable Sector Check
