Base pay range$13.66/hr – $14.47/hr
Live Long Well Care of Winston‑Salem is now accepting applications for PT/PRN CNA/Caregiver/Companions on all shifts, weekend availability preferred. This position provides personal care and companionship to seniors. Day and night positions are available, as are weekends. hone-certified‑caregivers may receive a premium. Must be able to work some weekends, have a valid driver’s license and a clean driving record, and provide services to various addresses within a 30‑mile radius of the branch.
An up‑to‑date smartphone is required for electronic time and attendance, care tasks, and communication.

This position reports to the CSM (Client Services Manager) or designated supervisor and is synonymous with the positions of Companion, Housekeeper, Homemaker, and Chore Worker.
Principal Duties and Responsibilities- Assist with ADLs (Activities of Daily Living): bathing, dressing, ; feeding. Mobility assistance including the use of transfer aids (Hoyer lifts, gait belts, etc.) and transferring & positioning.
- Meal preparation & cooking, light housekeeping & laundry (washing dishes, cleaning theDd kitchen after meals, laundering clothes, bed linens, etc.).
- Grocery shopping/errands/transportation, working with dementia/Alzheimer’s clients.
- Provide companionship while engaging in daily activities.
- Pet services: grooming, feeding, walking.
- Represent the company positively; promote safety & wellbeing for clients and employees.
- Maintain a clean, safe, and orderly environment for clients and families.
- Document services accurately and submit in a timely manner.
- Immediately report any changes in client needs or incidents to the supervisor.
- Respond to client needs sensitively and promptly.
- Attend and participate in internal staff development programs; obtain continuing education as required. Maintain and enhance clinical practice skills.
- Perform other duties as assigned in response to client, office, or facility needs.
- Adhere to HIPAA guidelines.
- Report for work on time, prepared to assume duties.
- Communicate effectively, both verbally and in writing.
- Have strong social work skills and connect with individuals of all ages.
- Adhere to dress code and display company badge.
- Comply with company policies and procedures.
- Assist or direct individuals to appropriate resources with proper follow‑through.
- Meet all requirements related to risk management, safety, infection control, TB screening, security, and fire.
- Assume responsibility for mandatory in‑services, certification, licensure, and competencies.
- Education
:
High school graduation or GED required. - Completed any required state‑regulation training.
- Experience (desired):
Two years in a similar position and/or with the elderly. - Competencies
:- Read, write, speak, and understand English fluently.
- Work with minimal supervision.
- Conduct all business professionally and confidentially.
- Pass competency testing on hire and ongoing: meal prep, housekeeping, infection control, safety, emergencies, transport, pet care.
- Have an updated Android or iPhone.
- Valid driver’s license with proof of insurance.
- Reliable transportation; commute to client locations.
- Physical ability to lift, bend, transfer, and perform ADLs; sit or stand for extended periods.
- Work outdoors in daylight and light traffic is not required (wheelchairs and *).
- 50‑90% of work performed in client homes; may involve exposure to bodily fluids or cleaning agents.
