- Location: Baptist Medical Center

As a skilled and compassionate RN, you will play a pivotal role in our healthcare team, leading the coordination of patient care from assessment to evaluation. Job Responsibilities- Patient/Family Centered Care: Utilize the nursing process to deliver compassionate care, placing patients and their families at the center of every decision. Conduct thorough assessments, develop nursing diagnoses, identify outcomes, plan interventions, and coordinate care to promote optimal health and wellbeing in accordance with the policies of WFBH.
- Patient Discharge & Referrals: Participate in multidisciplinary discharge planning referrals to hospital and community resources as needed.
- Safe, Therapeutic, and Healing Environment: Create a secure and healing environment by safeguarding patient privacy and ensuring responsive, respectful, accepting, and professional behavior. Embrace technology and equipment to enhance patient and staff safety.
- Effective Communication: Demonstrate excellent communication skills to foster collaboration among the healthcare team, facilitating information sharing throughout the continuum of care. Active participation in our professional practice model and shared governance initiatives will promote teamwork, leadership, and clinical excellence.
