About the Role:
Tidewell Hospice is seeking a Social Worker to support patients and families during their end-of-life journey. This role provides psychosocial support, care coordination, and resource assistance to ensure comfort and dignity for those we serve.
Key Responsibilities:- Assess patient and family needs related to coping, mental health, financial resources, and caregiver support.
- Provide individual and family counseling, crisis intervention, and bereavement support.
- Collaborate with the Interdisciplinary Group (IDG) to develop and manage patient care plans.
- Assist with financial resources, Medicare/Medicaid, and long-term care planning.
- Maintain accurate electronic medical records (EMR) and participate in IDG meetings.
- Support patients and families with advance directives and memorial planning.
- Educate the community on hospice care.
- Participate in an on‑call rotation.
- Education & Experience: Minimum one year of social work experience in a healthcare setting (internships may qualify).
- Licensure: Must hold one of the following Florida licenses or be License eligible (Registered Intern) within 90 days of hire:
- Social Work (LCSW) or Social Work (RCSWI)
- Mental Health Counselor (LMHC) or Mental Health Counselor (RMHCI)
- Marriage and Family Therapist (LMFT) or Marriage and Family Therapist (RMFTI)
- Other Requirements:
- Valid Florida driver’s license and reliable transportation for field‑based roles.
- Proof of valid auto insurance.
