Bay Care Medical Group is an experienced physician-led practice seeking an Advanced Practice Provider to join our team in Winter Haven.

Our outpatient office is open Monday‑Friday from 8 am‑5 pm and provides care to patients 6+ years old.

Provide comprehensive primary care to adult patients, including history taking, physical examinations, diagnosis, treatment planning, and follow‑up care. Work independently to deliver high‑quality patient care and communicate effectively with patients, families, and multidisciplinary teams.

• Minimum two years’ experience as an Advanced Practice Provider in a primary‑care setting.
• Excellent communication and interpersonal skills.
• Comfortable working independently and within a small team.
