Key Responsibilities Include- Providing skilled nursing care in patients' homes, ensuring outstanding clinical outcomes
- Utilizing electronic documentation via tablets in patients' homes
- Collaborating with a multidisciplinary team to deliver personalized care
- Active, unencumbered LPN/LVN license in the practicing state; compact licenses must transfer within 90 days
- Current CPR certification with hands‑on component
- TB skin test completed within the last 12 months
- Reliable transportation
- Preferred but not required: six months of hands‑on nursing experience
- Delivering compassionate care tailored to individual patient needs
- Adhering to healthcare regulations and safety standards
- Supporting patient well‑being through clinical expertise
