Previous experience ofworking in General Practice is desirable; however, we are able to provide aTransition Programme for those who are new to Primary Care, if required.
Main duties of the jobThe post holder will work as an autonomous clinician within the GP practice, providing advanced clinical care to patients presenting with acute,chronic, and complex health needs. The role requires independent prescribing,advanced clinical assessment skills, and the ability to manage patients across their lifespan within a primary care setting. The ANP/ECP will work collaboratively with GPs, nurses, and the wider multidisciplinary team toensure safe, effective, and patient-centred care, contributing to service development and quality improvement within the practice.
About usWe pride ourselves in providing the highest quality of traditional General Practice carein a supportive team environment. We are a friendly team consisting of 3 GPs, 7
ANPs, 1 ECP, 4 Practice Nurses, 1 NA, 2 HCAs, 1 GPA and a supportive admin staff caring for 14,500 patients in a semi-rural setting. We are also supported by PCNARRS staff including Clinical Pharmacists, First Contact Physiotherapist,Social Prescriber and Health Coaches.
Main Duties and Clinical Responsibilities
Undertake comprehensive clinical assessments, including history taking, physical examination, and diagnosis
Manage undifferentiated and differentiated presentations independently assess,diagnose, treat, and discharge or refer patients as appropriate
Prescribe medications independently in line with clinical guidelines and best practice
Manage acute, urgent, and long-term conditions
Order, interpret, and act upon diagnostic tests and investigations
Provide health promotion, disease prevention, and patient education
Provide same-day urgent care appointments and routine clinical appointments as required
Refer patients to Secondary Care, community services, or allied health professionals where appropriate
Ensure continuity of care through appropriate follow-up and safety-netting
Maintain accurate, clear, and contemporaneous clinical records
Multidisciplinary working
-work collaboratively with GPs, practice nurses, HCAs, pharmacists, andthe wider PCN team Participate in MDT meetings and clinical discussions Support and mentor junior staff where appropriate
Quality, governance, and safety work within practice policies, procedures, and clinical governance frameworks, adhere to safeguarding policies for children and vulnerable adults
Participate in clinical audit, quality improvement, and service development initiatives
Maintain professional registration, revalidation, and mandatory training
Personal Professional Development Maintain and develop clinical competence through CPD Keep uptodate with evidence-based practice and national guidance
Actively contribute to a learning and supportive practice environment
Responsibility for Information Governance Ensure patient confidentiality is maintained at all times comply with GDPR,data protection, and Caldicott principles
Use clinical systems appropriately and securely
Health& Safety Work in accordance with Health & Safety legislation and practice policies report incidents, near misses, and risks appropriately.

Person Specification Qualifications- Registered Nurse with the NMC
- Qualified ANP
- Independent prescriber
- Managing acute and same-day presentations
- Routine patient reviews and clinics
- Supporting long-term condition management
- Ability to assess and manage patient risk effectively and safely
- Well developed word processing/data collection/IT skills
- Excellent interpersonal, verbal and written communication skills
- Time management and ability to prioritise workload
- Able to analyse data and information, drawing out implications for the individual patient/impact on care plan
- Able to establish and maintain effective communication pathways within the organisation, the local PCT and with key external stakeholders
- Experience of use of Systm One
- Proven record of effective use of networking and influencing skills
- Ability to think strategically
- Experience of presenting information to wider audience
- Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance.
- Understanding of systems to gain an understanding of the health needs of the practice population as they relate to primary care
- Understanding of evidence based practice
- Knowledge of national standards that inform practice (eg, NICE guidelines etc)
