We are looking for an experienced, enthusiastic& compassionate Registered Nurse with an interest in Diabetes to join our clinical team. We are a happy established clinical team that is looking for theright nurse to join us.
Previous experience of working in General Practice is desirable; however, we are able to provide a Transition Programme for those whoare new to Primary Care, if required.
Main duties of the jobThe role will include working with our existing diabetic team to conduct annual reviews and promote a patient centred approach to diabetic care, as well as working as a Practice Nurse with general nursing responsibilities.

Practice Nurses, 1 NA, 2 HCAs, 1 GPA and a supportive admin staff caring for 14,500patients in a semi-rural setting. We are also supported by PCN ARRS staff including Clinical Pharmacists, First Contact Physiotherapist, Social Prescriber and Health Coaches.
Main duties of the job
- Deliver evidence-based nursing care within your scope of practice
- Support long-term condition management of diabetes (and associated conditions) and run clinics.
- Support public health and screening programmes, including: immunisations & vaccines, cervical screening
- Health promotion and prevention
- Provide wound care and routine nursing procedures
- Contribute to our research programmes
- Contribute to QOF, enhanced services and practice targets
- Maintain accurate clinical records
- Teach and support patients to self-manage their conditions
- Participate in clinical audits, quality improvement and governance activity
- Liaise with external services to ensure patients receive appropriate care
- Respond appropriately to clinical emergencies and incidents
- Registered Nurse with the NMC
- Experience of chronic disease management - Diabetes
- Ability to work autonomously and as part of a multidisciplinary team
- Excellent communication, organisational and time-management skills
- Commitment to ongoing professional development and revalidation
- Experience working as a Practice Nurse or Community Nurse
- Strong clinical skills including immunisations & wound care
- Good understanding of safeguarding adults and children
- Knowledge of GP systems, QOF and enhanced services
- Diabetic Diploma or equivalent (training can be provided)
- An understanding, acceptance and adherence to the need for strict confidentiality.
- Ability to use own judgement, resourcefulness and common sense.
- Ability to work without direct supervision and determine own workload priorities.
- Ability to work as part of an integrated multi-skilled team.
- Pleasant and articulate.
- Able to work under pressure.
- Able to work in a changing environment.
- Able to use own initiative.
- Experience of working in accordance with the NMC Code of Professional Conduct and The Scope of Professional Practice.
- Experience of Microsoft Office software.
- Experience of dealing with the public/patients.
- Experience of Systm One computer system
- Flexibility of working hours/ able to work at the desired times
- Experience of working within Primary Care
- Registered General Nurse.
- A demonstrable commitment to professional development.
- Competent in basic nursing duties required for the post.
- Specific nursing skills / qualifications.
