$19.00/hr – $23.54/hr

• Provides quality care to guests in a nourishing environment.
• Provides individualized attention to encourage each guest’s ability to maintain or attain the highest practical physical, mental, and psychosocial well‑being.
• Assists with daily living activities (bath, dress, groom, toileting, feeding).
• Attends to individual needs of all guests in regard to incontinent care, transferring, ambulation, ROM, communication, and other needs.
• Maintains skin integrity to prevent pressure ulcers, skin tears through changing, turning, repositioning, and applying moisturizers.
• Performs tasks assigned by the floor nurse (vital signs, weighing, creams/ointments, specimen collection).
• Knowledgeable of individual care plans; supports planning by reporting observations and behavioral changes.
• Assists the Wound Care Nurse as directed.
• Completes documentation per nursing policies, maintaining confidentiality.
• Ensures infection control and standard precautions when providing care and uses equipment safely.
• Reports hazards, damaged equipment, and supply issues to supervisor.
• Responds promptly to call lights.
• Assists with new admissions, readmissions, and transfers within the facility.
• Communicates effectively with guests, visitors, families, peers, and supervisors.
• Attends and participates in departmental meetings and in‑services.

• Customer service oriented.
• Certified Nursing Assistant (CNA) or qualified as a Nursing Assistant Registered (NAR).
• Experienced in long‑term care (preferred).
• Communicates well in both verbal and written forms.
• Committed to professional and caring interaction toward residents, families, and staff.

• Certified/registered as a nursing assistant in Wisconsin.
• Experience in long‑term care.
