Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead and coordinate compassionate end-of-life care in patients' homes across Wisconsin Rapids. You will conduct comprehensive assessments, develop and update care plans, and work within an interdisciplinary team to ensure individualized, comfort-focused support for patients and families.
Ideal candidates have at least one year of RN experience, preferably in hospice or home health, with current licensure and CPR certification.
