Youth Services Mentor-Nights and Weekend

Responsibilities

• Teach others daily living skills, social skills, and community involvement.
• Assist and support children and youth with disabilities to become actively involved in their communities, develop friendships, and use community resources independently.
• Provide opportunities for inclusion through planned activities, social interactions, exposure, advocacy, training, and education as identified in the assessment and planning process.
• Assist with personal care tasks as needed (e.g., mobility, eating, bathroom).
• Document all individual services provided daily in the required time frame and format accurately.
• Maintain open, respectful, and clear communication with the participant, their families, and the supervisor.
• Meet all necessary training requirements.
• Transport and accompany individuals to community activities in company or personal vehicles.
• Pass a Wisconsin caregiver background check.
• Hold a valid driver’s license.
• Work a flexible/non‑standard schedule, including some evenings and weekends.
• Use a company email address and download an application to your personal cell phone for multi‑factor authentication (MFA) to ensure cybersecurity.
• Provide a reliable vehicle (unless exempt with HR).

• Minimum of 1 year of work experience with individuals and/or children with disabilities.
• Excellent written and verbal communication skills.
• Knowledge of the modifications, adaptations, and use of specialized equipment.
• General knowledge of computer use and programs required.
