Position Summary

This position description has been prepared to assist in evaluating duties, responsibilities, and skills of the position. It is not intended as a complete list of specific responsibilities and duties, nor is it intended to limit duties to those listed.

This position requires planning, developing, implementing, and evaluating services for individuals within the confines of the WI Statutes and Administrative Codes of the State Department of Health Services. Employee exercises independent judgment and discretion within established guidelines.

Essential Duties and Responsibilities
  • Responsible for services through the Wisconsin Birth to Three Program and the Children’s Long-Term Support Program (CLTS) which is defined by the Federal and State guidelines and requirements.
  • Conducts intake and assessments for the Wisconsin Birth to Three Program and the Children’s Long-Term Support Program (CLTS).
  • Assisting in determining eligibility for the Wisconsin Birth to Three Program and the Children’s Long-Term Support Program (CLTS).
  • Coordinates enrollment, develops, implements, and reviews service plan, for both the Wisconsin Birth to Three Program and Children’s Long-Term Support Program.
  • Provides on-going monitoring/case management to recipients (eligible children) and their families.
  • Coordinates and oversees services for children in the Wisconsin Birth to Three Program and the Children’s Long-Term Support Program (CLTS). Acts as liaison between potential contracted agency and county.
  • Provides expertise on navigating the early intervention system using family centered practices, linking families to community resources, fostering strong family professional partnerships.
  • Service coordinators coordinate the delivery of all services across agency lines and connect children and their families with early intervention and other services needed as identified in the IFSP.
  • Ensure families are informed, involved, and consulted during their child’s enrollment.
  • Coordinating, facilitating, and participating in team activities.
  • Coordinating and monitoring the delivery of an array of services to address each child’s unique needs and circumstances as outlined in the Individualized Family Service Plan.
  • Ensures state and federal compliance with the all the Long-Term support programs.
  • Maintains individual consumer files. Maintains documentation, case notes, and provider notes for the ongoing case management within the long-term support programs. Maintains &updates all files, reviews cases as mandated and necessary for compliance.
  • Reviews monthly expenses/claims, and authorizations for services.
  • Prepares and completes consumer information for data entry on the state and county platforms for reporting such consumer information, as required by state and county program guidelines.
  • Authorizes services for consumers based upon assessed need within the limits of available funds, following allowable services guidelines, and following state and federal guidelines for the programs.
  • Implements and monitors Service Agreements for contracted services through the Long-Term Support Programs. Reviews and approves monthly claims for such services.
  • Reviews and follows policies and procedures to ensure compliance with Federal, State, and County laws and regulations.
  • Writes internal policies/procedures when appropriate for programs in the unit.
  • Contacts State offices and other professionals to collaborate and maintain compliance with programs.
  • Develops new resources, financial and/or service delivery systems.
  • Speaks to community groups regarding the services available through the Long-Term Support Unit.
  • Assists with rotational on call for after hours for CH. 51 and Ch. 55’s, if employee meets requirements.
  • And all other duties assigned by the unit supervisor.
