Job Overview
A Skilled Nursing Facility in Thorp, Wisconsin is seeking an experienced Licensed Practical Nurse (LPN) for an evening-shift travel assignment. This role is ideal for an LPN who prefers evening hours and is comfortable providing consistent, resident-focused care in a long-term care environment.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing setting
• Administer medications and treatments per provider orders
• Monitor resident conditions and promptly report changes
• Assist with activities of daily living and coordinate care with CNAs
• Complete accurate and timely documentation
• Collaborate with nursing staff and interdisciplinary care teams
• Ensure resident safety and compliance with facility policies and procedures

Requirements

• Active LPN license in Wisconsin or compact state (if applicable)
• Skilled nursing or long-term care experience preferred
• Ability to work evening shifts reliably
• Strong communication, organization, and documentation skills
