. Our growing home health agency is seeking a compassionate and experienced full‑time RN Case Manager to coordinate and oversee patient care in the home setting, ensuring personalized, high‑quality outcomes for every client.

Transitions At Home Registered Nurse Case Managers provide skilled nursing care that includes assessments, medications, treatments, and coordinating services. They counsel the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager and Administrator.

• 40 hours per week, 8:00 am – 4:30 pm
• Monday through Friday
• No night shifts
• Limited on‑call rotation

• Active Wisconsin Registered Nurse license
• Two (2) years’ nursing experience, 1 year in home health (preferred)
• CPR certification
• Valid Wisconsin driver’s license and car insurance
