Meadowbrook at Black River Falls is seeking a Wellness Director to lead the day‑to‑day wellness operations of our 24/7 Residential Care Apartment Complex (RCAC).
This is a hands‑on leadership role responsible for overseeing resident care, supporting and developing our Resident Assistant team, coordinating staffing, maintaining compliance, and ensuring our residents receive consistent, compassionate care across all shifts.
The ideal candidate is a strong communicator and organized leader who enjoys being involved in the day‑to‑day operations of a community and is comfortable balancing resident needs, staff support, family communication, and regulatory requirements.
What You'll LeadAs the Wellness Director, you will provide leadership and oversight for the wellness department and Resident Assistant team while helping ensure a safe, comfortable, and supportive environment for our residents.
Key responsibilities include:
Oversee the day‑to‑day wellness operations of our 24/7 RCAC community.
Lead, support, and develop the Resident Assistant team across all shifts.
Coordinate staffing and schedules to ensure adequate coverage and consistent resident care.
Monitor resident needs and changes in condition and ensure appropriate follow‑up and communication.
Assist with resident assessments, service plans, care documentation, and ongoing monitoring of resident needs.
Partner with residents, families, providers, and the interdisciplinary team to address concerns and coordinate care.
Provide ongoing training, coaching, and education to Resident Assistants.
Ensure staff follow established policies, procedures, and state regulations related to resident care and medication management.
Monitor medication systems and assist with medication management, audits, and staff competency as applicable.
Participate in quality assurance and performance improvement initiatives.
Maintain a strong focus on regulatory compliance and survey readiness.
Respond to staffing and resident care needs as they arise and provide leadership support across shifts.
Participate in an on‑call/Manager on Duty rotation as needed.
Assist with hiring, onboarding, performance management, and retention of wellness staff.
Promote a positive team culture built on accountability, communication, compassion, and respect.
Maintain confidentiality and protect the privacy, dignity, and rights of every resident.
What We're Looking ForWe are looking for someone who is a confident and compassionate leader who can build strong relationships with both residents and staff.
Qualifications include:
Current Wisconsin RN or LPN license in good standing.
Previous leadership, supervisory, or healthcare management experience preferred.
Experience working in assisted living, RCAC, long‑term care, or a similar senior living environment preferred.
Strong understanding of resident care, medication management, staffing, and regulatory requirements.
Excellent communication, organization, and problem‑solving skills.
Ability to lead a team while remaining hands‑on and approachable.
Ability to work collaboratively with residents, families, providers, and other departments.
Ability to respond to changing resident and staffing needs in a 24/7 environment.
A Typical DayNo two days are exactly the same in the Wellness Director role. Your day may begin with rounding on residents, checking in with staff, and reviewing any overnight concerns or changes in resident needs.
You may spend part of your day reviewing staffing and schedules, completing resident assessments or care documentation, following up with families, or working with staff on training and performance. You’ll also help ensure medications, documentation, care plans, and daily processes are being completed appropriately.
