Allay Hospice is seeking a dedicated Hospice RN Case Manager to provide patient-centered care across the assigned territory. This full-time role combines clinical leadership with collaborative teamwork to deliver compassionate hospice services in patients' homes.
The position requires an accredited nursing degree and state licensure, with 1 year of nursing experience (3+ preferred) and hospice experience encouraged. A valid driver's license and insured vehicle are required for travel.
