The RoleTo provide a safe and effective patient focused specialist clinical pharmacy service to a specified patient group(s) supporting medicines governance and quality improvement activities within specialist area(s) to ensure safe, effective and patient-centred systems in place.
In This Key Role, You WillTo deliver patient-centred specialist pharmaceutical care to patients in specified clinical area(s). To communicate effectively, placing the person at the centre of any interaction. To deliver patient education to ensure the safe and effective use of medications and devices. To collaborate with the wider pharmacy and multidisciplinary team; actively participating in consultant ward rounds and multidisciplinary team meetings.
What You’ll Bring- Masters of Pharmacy Degree
- Registered member of General Pharmaceutical Council
- Independent prescribing qualification or must work towards
- Evidence of postgraduate training and professional development in clinical pharmacy practice e.g. Post Registration Foundation Framework/ 1st year clinical diploma
- Relevant post registration experience in clinical pharmacy.
- Ability to effectively monitor patients’ therapeutic regimens in order to ensure effectiveness and reduce toxicity
- Ability to work effectively as part of a team, in a pharmacy and multi-disciplinary environment.
- Commitment to revalidation as per directions of General Pharmaceutical Council
