Responsibilities
The primary responsibility of the Social Worker in Adult Social Care is to conduct assessments of individuals under the Care Act who present with diverse and complex needs. The role involves collaborating with these individuals to develop suitable support plans that address identified outcomes and effectively manage associated risks. The aim is to make a real difference by using a strength-based approach to create opportunities, build resilience, and establish long-term support networks for people to live independently and safely, working in compliance with the Care Act 2014, Mental Capacity Act 2005, and Mental Health Act 1983, including the ability to provide tailored information and advice to individuals, families, and carers in accordance with their specific circumstances and needs.
ProgressionThe post holder has opportunity to work within the competency framework to achieve progression to Advanced Social Worker, Grade 11 following completion of their ASYE and ability to evidence advanced knowledge and skills via the Progression Pathway.
QualificationsThe post requires professional registration to complete statutory duties. A newly qualified Social Worker is expected to commence the Assessed and Supported Year in Employment (ASYE) on entry to the role and complete it within the first 12 months of employment.
Location and ScheduleThis role sits in the Learning Disabilities team. The role is offered on a hybrid basis with 2 days a week office based.
