Position: Social work job - Oxfordshire - Moving into Adulthood

Your new company
We are looking for experienced Social Workers to join our Moving into Adulthood Team within Adult Social Care. This role focusses on supporting young people aged 18-25 as they transition into adulthood, helping them achieve their outcomes through a strength-based approach.

Your new role

  • You will work collaboratively across adult and children's services, health, education, and voluntary sectors to build resilience and long-term support networks, enabling individuals to live independently and safely.
  • Deliver services in line with legislation, including the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, Children and Families Act 2014, Equality Act 2010, and Autism Act 2009.
  • Undertake strength-based assessments, reviews, and interventions.
  • Develop support plans that reflect individual needs and outcomes.
  • Promote independence through reablement services and assistive technology.
  • Build strong working relationships with SEN teams, schools, colleges, health services, and community organisations.
  • Manage a caseload, conduct face-to-face visits, and participate in duty work, including emergency advice.
  • Identify and report safeguarding concerns and carry out enquiries in line with the Care Act.
  • Maintain accurate, defensible records and contribute to service development.

What you'll need to succeed

  • Recognised Social Work qualification and current registration.
  • Experience in adult social care and supporting young people transitioning into adulthood.
  • Knowledge of SEN, education settings, and strength-based approaches.
  • Strong communication, assessment, and report-writing skills.
  • Ability to work independently and within multidisciplinary teams.
