Are you a qualified Social Worker with experience working within a hospital or healthcare setting? We are currently looking for an experienced and compassionate Hospital Social Worker to join a dedicated team supporting patients and their families during what can often be a challenging time.

The Role As a Hospital Social Worker, you will work closely with patients, families, healthcare professionals and external agencies to assess needs, provide support and help ensure patients receive the appropriate care and services following discharge. Your responsibilities will include:
Completing social care assessments and reviews. Supporting patients with discharge planning and transition from hospital into appropriate care settings. Working closely with multidisciplinary teams, including doctors, nurses, occupational therapists and other professionals. Assessing safeguarding concerns and taking appropriate action where required. Supporting patients and families with complex social and emotional needs. Liaising with local authorities, care providers and other external agencies.

Maintaining accurate and up-to-date case records. Ensuring all work is completed in line with relevant legislation, policies and safeguarding procedures. About You We are looking for someone who:
Holds a recognised Social Work qualification and is registered with Social Work England . Has experience working within a hospital, NHS, community or adult social care environment. Has strong knowledge of safeguarding, assessment and care planning. Is confident working as part of a multidisciplinary team. Has excellent communication and organisational skills. Can manage a varied caseload and work effectively in a busy environment.
