Salary Range: $45,000–$55,000
Sign-On Bonus: $500
Job responsibilities
:
To assure the highest quality of services to the people in our day habilitation programs. All program participants will enjoy clean, safe and well-organized programs while participating in activities that are meaningful, personalized and therapeutic. All staff must be trained and supported to provide outstanding services that support the organizational mission. The person in this position must have the ability to work independently with minimal supervision, be organized, efficient, and flexible as well as possess good time management skills.
He/she will coordinate all aspects of the Day Hab Systems reporting standards to ensure organizational compliance with all DMA and DDS regulations and have the ability to teach others and provide constructive feedback and supervision. He/she must demonstrate excellent verbal/written communication with a proven ability and familiarity with various data applications (Crossroads, Microsoft, HCSIS) and possess the ability to work in an energetic, creative and positive manner with others.
Must adhere to all NuPath policy and procedures.
BA in relevant field; 5 years' experience Day Hab/DMA within Human Services may be substituted in lieu of degree.
Day Hab Systems Management- Provides supervision in all areas of the Day Hab Systems reporting standards to ensure organizational compliance with all DMA, DDS regulations.
- Provides leadership for the ongoing conversion of existing case records/files into the online digitized format.
- Must be willing to work closely with program participants (Adults with developmental disabilities).
- Must have experience with the ISP process.
- Responsible for preparation and coordination of assessments/reports from consultants and other team members.
- Oversee the implementation and monitoring all tracking systems related to data reporting requirements.
- Must have experience with Day Hap (MA Health) system and DDS (HICSIS)
- Provide input in programs and preparation for CARF, DDS survey and annual DMA audits.
- Assumes a leadership position in the organization.
- Communicates effectively with all necessary parties to assure program participants are receiving the community services.
