RN Case Manager

Stellar Health Group is seeking a Registered Nurse to serve as a Case Manager in a geriatric, sub‑acute care setting.

Full‑time, $75,000 – $150,000 annually (dependent on experience)

• Current unencumbered State of Massachusetts RN license
• Knowledge of Medicare, Medicaid, PPS, PDPM, MDS and third‑party payment
• Knowledge of regulatory agencies and licensure standards
• Minimum 5 years’ experience working in geriatrics with sub‑acute experience
• Strong clinical background
• Case management experience strongly preferred

• Monitor level of HMO patients on admission and throughout stay
• Obtain prior approval for all HMO patients for transportation, therapy, etc.
• Track HMO and Medicare days
• Obtain pre‑authorization for special services and equipment as needed
• Ensure sound clinical and financial outcomes
