Benchmark at Woburn is looking for a compassionate Licensed Practical Nurse (LPN) to join our team! As an LPN, your main role will be to deliver nursing care to our residents within a warm, home-like environment. • Assess, communicate, and document residents’ status accurately.
• Report significant changes in resident health and behavior to the appropriate personnel per regulations.
• Supervise the daily work of Resident Care Assistants.
• Mentor and guide Lead Resident Care Assistants.
• Coordinate care with community providers through effective case management.
• Support resident independence while meeting service demands.
• Recognize and report elder abuse, neglect, and exploitation per regulations.
• Respond to emergencies appropriately and know when to call for backup.

• Current state LPN license and CPR certification.
• Graduate of an approved LPN program.
• 1-2 years of nursing experience preferred.
• Experience working with the elderly is highly desirable.
