Registered Nurse Case Manager – Woburn, MA

Base pay range – Woburn Nursing and Rehab Center – Woburn, MA

• Monitor HMO and Medicare patients from admission through discharge.
• Obtain prior approvals for HMO patients for transportation, therapy, and other services.
• Track HMO and Medicare days accurately.
• Obtain pre‑authorization for special services and equipment as needed.
• Ensure sound clinical and financial outcomes for residents.
• Attend family meetings to communicate care plans and updates.
• Collaborate with interdisciplinary teams to promote resident‑centered care.
• Maintain compliance with regulatory agencies and licensure standards.
• Support staff in understanding and implementing payment and case‑management procedures.

• Current, unencumbered Massachusetts RN license.
• Strong clinical background with at least 5 years of experience in geriatrics, including sub‑acute care.
• Knowledge of Medicare, Medicaid, PPS, PDPM, MDS, and third‑party payment processes.
• Familiarity with regulatory agencies and licensure standards.
• Case‑management experience strongly preferred.
• Excellent communication and critical‑thinking skills.
