De gemeente Woerden zoekt een welzijnscoach die inwoners laagdrempelig ondersteunt, vragen helpt verduidelijken en hen naar passende zorg verwijst. Je werkt aan zelfredzaamheid, onderhoudt verbindingen met vrijwilligers en zorgpartners en beheert zelfstandig je caseload.
Je werkt vanuit Woerden en kan ook op Oudewater of vanuit huis aan de slag. De functie vraagt een HBO-opleiding in SPW/MWD, minimaal 24 uur per week beschikbaarheid en ervaring in het sociaal domein.
