Reinaerde zoekt een Begeleider of Regiebegeleider voor een woonlocatie in Woerden. Je biedt maatwerk zorg en begeleiding aan volwassenen met NAH en lichamelijke beperkingen. Je werkt in een hecht team en coördineert zorg in samenspraak met familie en disciplines.
Je voert ADL-taken uit, begeleidt bij structuur en vrije tijd, en documenteert voortgang. Een Verklaring Omtrent Gedrag is vereist; werkdagen zijn wisselend met avond- en weekenddiensten.
