We are looking for a friendly, organised and professional
Patient Care Coordinator to join our busy and supportive practice team.
This is a key patient‑facing role, providing front‑of‑house support and coordinating patient access to clinical services. You will be the first point of contact for many of our patients, helping ensure they receive timely, safe and appropriate care.
Working Pattern: Part‑time 2 days per week Monday and Friday, 08:00-18:30
Main duties of the job- Welcoming patients and visitors in a professional and compassionate manner
- Answering and managing incoming telephone calls
- Booking, cancelling and managing appointments using EMIS Web
- Signposting patients to the most appropriate clinician or service
- Managing patient queries and ensuring confidentiality at all times
- Supporting clinicians and the wider multidisciplinary team
- Maintaining accurate patient records
Heathcot Medical Practice is a well‑established GP practice serving approximately 17,500 patients in Woking, Surrey.

Monday & Friday 08:00‑18:30 Purpose of the Role
To provide high‑quality reception and administrative support to ensure the smooth running of the practice and safe coordination of patient care.
Key Responsibilities Patient Access & Communication- Act as first point of contact for patients via telephone and face‑to‑face
- Manage non‑GP appointment bookings
- Signpost patients appropriately according to practice protocols
- Handle sensitive information with discretion
- Maintain strict patient confidentiality
- Follow practice policies and procedures
- Support safe systems of work in line with CQC standards
- Work collaboratively with clinicians and administrative colleagues
- Support service improvements where required
- Experience working in a customer service or reception environment
- Experience handling telephone queries
- Experience of working in primary care
- Experience of working in a GP practice
- Excellent communication and interpersonal skills
- Ability to work under pressure
- Ability to prioritise workload
- Understanding of confidentiality and data protection
- Previous NHS or GP practice experience
- Experience using EMIS Web
- Understanding of primary care services
- Experience in care coordination or patient navigation
