Overview

This is a dynamic and autonomous community-based role at the forefront of palliative and end of life care across Northwest Surrey. You will work in close partnership with the Nurse Consultant, contributing to day-to-day clinical leadership and service development, while supporting and developing colleagues within a collaborative, multidisciplinary environment. As an expert practitioner, you will undertake holistic assessments, provide specialist advice, and deliver high-quality, evidence-based care that improves the experience of patients and their families at every stage of the palliative journey.

You will also play a key role in education and workforce development, building capability across community nursing teams and partner organisations. The role combines clinical leadership with service development, quality improvement, and governance responsibilities, alongside delivering care and support across a range of community settings. You will be instrumental in driving innovation, strengthening partnerships, and striving for excellence in palliative care.

Main duties of the job
  • Act as a professional role model to all staff, both internal and external, working with multi disciplinary teams, taking the lead and developing pathways to support palliative care delivery.
  • Use specialist assessment skills, nursing diagnosis and interpretation skills for complex patient conditions to determine the appropriate course of action/plan of care.
  • Provide specialist advice and support as requested on all aspects of palliative care.
  • Demonstrate clinical expertise and be accountable for holistic assessments of patients referred to the service.
  • Assess and manage own patient caseload of complex cases.
  • Treat all patients as individuals and develop patient centred care plans.
  • Communicating with and involving, supporting, informing and educating patients, families and carers.
  • Providing programs of education to other CSH employees on palliative care.
  • Analyse and act appropriately in complex situations and escalate where required.
  • Recommend a prescribed course of treatment/care using research based evidence and principles of best practice for the patient.
