About the RoleWe have a fantastic opportunity for a SEND Casework Practitioner to join our SEND team on a permanent, full time basis. As a SEND Casework Practitioner you will develop and maintain Education Health and Care Plans from initial application and assessment through to the annual review. You will work with families of children and young people with Special Educational Needs and Disabilities in a highly person‑centred relational approach, coordinating service provision across education, health and care to ensure holistic delivery of plans for individual children and families.

- Improvement of the SEND Service for pupils and families, promoting the Local Authority’s values and collaborating with the rest of the SEND Service.
- Work collaboratively with families, schools and key partner agencies to ensure appropriate provision for individual children and young people with SEND.
- Engage with educational settings and relevant practitioners to support, advise and monitor the local offer, working with schools on individual cases as needed.
- Manage a case load of children and young people with SEND; act as a key point of contact, engaging with families, children, schools, colleges and partners where statutory assessment requests have been made or circumstances have changed.
- Prioritise statutory processes for vulnerable children and young people, ensuring appropriate provision is identified without delays.
- Understanding of SEN legislation and procedures, including Children’s Act, Code of Practice, DDA and more.
- Experience working with children and/or families with SEN.
- Experience managing complex casework requiring mediation or conflict resolution in an educational environment.
- Ability to write detailed and complex reports independently using information from internal and external colleagues.
- Good IT skills, including Microsoft Office and database systems.
- Willingness to travel to various locations within the borough to develop relations with educational settings and attend training to key stakeholders.
