Overview
Provide clinical support, assistance, and professional expertise to all callers of the Community Nursing Triage Telephone Support. This will range from signposting the referral to the appropriate service, making a direct referral, or offering advice. Work in conjunction with administrative support staff to provide high quality, clinical triage using knowledge, skills, critical thinking, and professional judgment. Control and manage calls effectively by using skilled questioning, active listening, and excellent verbal communication skills in a calm, professional manner.
Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies. A key member of the Community Team, providing robust clinical services for patients, maintaining positive communication links with GP practices and other members of the Multi-Disciplinary Team, and providing high quality patient-focused care using evidence-based practice.
- Provide clinical support and advice to callers of the Community Nursing Triage Telephone Support.
- Signpost referrals to the appropriate service, make direct referrals, or offer advice.
- Work with administrative support staff to deliver high-quality clinical triage.
- Control and manage calls using skilled questioning, active listening, and verbal communication.
- Maintain detailed, accurate and confidential patient records in line with Trust policies.
- Maintain positive communication links with GP practices and members of the Multi-Disciplinary Team.
- Provide patient-focused care using evidence-based practice.
- Registered Nurse with experience in community nursing or clinical triage.
- Excellent communication and problem-solving skills.
- Self-motivated, forward-thinking, and autonomous.
- Adaptable approach to work, able to work within community nursing teams alongside other clinical staff.
- Access to a car and a valid UK driving licence.
- Desire to keep clinical skills updated and deliver a high-quality, locally based service to patients in the Wokingham locality.
Mon to Fri, 08:30-16:30 (approximately one weekend every 4-5 weeks and bank holidays in rotation). The Community Nurse service will also cover the 16:30-19:00 shift on a rota basis.
