Wokingham Borough Council is offering a permanent Social Worker role within the Primary Care Networks for Adult Social Care in Wokingham. This full-time position (37 hours per week) focuses on working with GP practices and partners to support adults and carers, preventing needs escalating and promoting wellbeing.
You will join a supportive team, with opportunities to work across health and social care, including home visits and community-based care, with hybrid working where available.
